Final Summary: On December 24, Firefighter Lionel Hoffer of the Milwaukee 
Fire Department died at a fire in a church when he fell through a hole in 
the second floor. Hoffer had been operating an attack line in the church 
with an engine company when he went to check a room for fire extension. 
His crew heard him call for help and found him hanging from a hole in the 
floor, but they were unable to keep him from falling. He fell approximately 
12 feet into the first floor. Rescue crews tried to reach him, alerted by his 
PASS device which had activated, but were hampered by the collapsed floor in 
the front of the building and barred security doors at the rear of the 
building. Rescue crews eventually fought their way through heavy heat and 
smoke conditions to Hoffer's location, removed him from the building, and 
administered emergency care. His air supply had been exhausted and his death 
has been attributed 
to smoke inhalation.
